The item is not bad in concept (read: it's better than 80% of the unreasonable thing they propose in this forum), but feel really a bit off in design.
For example i'd remove the bonus agility on insanity. I understand you've used Armlet as an ispiration, but armlet give strenght, and strenght give hp, that armlet in itself doesn't give (it give armor, damage and ias). In this item Agility give ias that would overlap with the ias the ability already give. Why not giving simply damage or more ias then? The users will understand better the real benefit of the item at first glance. Also, insanity is debatably worse than Berserk. 15% more damage on you is a lot, and i wouldn't exchange 20 agility for it, especially on non agi-heroes. If anything, i'd make the bonus damage lower, to give a real differentiating trait from berserk. Berserk make you take a lot of damage, this does not. Giving 20 more ias or 20 more damage feel less "different" than making it not damaging you, imho. Or even etter give it a damage numbing mechanic. You take 30% less damage now and then 30% more damage then ? Create a parallell with the old 30% bonus too, looks better overall (keep in mind that a "numbing" mechanic is REALLY STRONG, so you'd have to balance it differently).

• Items and recipes are more logical and intuitive
• Hammer is the old Javelin (the %chance fits naturaly in Basher + Mael)
