IIRC it's based on poison weapons and not shadow strike as you would think

It is indeed based on Shadow Strike.
The buff is physical, which means it will be removed by becoming ethereal but not by becoming magic immune or purged (cyclone and mirror image also remove it). However, it deals magical damage, so magic immunity will block the damage.
