Which spell creates the most?

Do Dummy Units have a specific Model?

There is no onoe spell that creates the most number of dummy units since most spells create a dummy unit for each target unit. So something like echoslam would create 20 dummy if there was 20 nearby targets.

Also some spells have two dummy units involved one for the art effect and one to cast the dummy spell.

I believe shiva would create the most units on average as it makes 36 dummy units every time and then an addition effect for every target unit.

Most dummy units are used to create a special effect, like chronosphere or shivas, however the dummy unit that casts most of the spells in dota (e00E) has the Line of sight blocker as its model, which has no real model.

The effect of Shiva's is provided by a single [h091] dummy unit which has the war3mapImported\Shiva's_Guard_FX model. And yes, it used to be provided by using 48 dummy units with the Abilities\Weapons\WingedSerpentMissile\WingedSerp entMissile.mdl model.
Plasma Field's effect is provided by attaching the effects\\Lightning_Ball_Tail_FX.mdx effect on Razor and a single dummy unit with the war3mapImported\Razor's_Plasma_Field_FX model, so there aren't many units involved either.

Under normal circumstances, Meat Hook should create the highest number of dummy units, otherwise - Scream of Pain or Death Pulse.
And yes, dummy units have their own models.

Actually, Hookshot creates more dummy units than Meat Hook does

Requiem of Souls creates up to 18 (15 to fire the Carrion Swarms, and 3 to cast Cripple in order to reduce the movement speed / damage)
The first ones are dependent on the amount of souls stored.
Poison Nova creates 19 (18 containing a Phoenix Fire based ability and having the Abilities\Weapons\ChimaeraAcidMissile\ChimaeraAcid Missile model and an [e00E] unit summoned by the spell itself as it is based on Summon Bear)

nope, those are just summoned units. A dummy unit is a unit created for triggered spells that either casts extra spells or is there for a visual effect. March of the Machines and Exorism are both hardcoded and thus have no dummy units involved.

The second one is a leftover from the times Echo Slam used to deal half echo damage from corpses. (as it required 2 dummy units with 2 different Fan of Knives based abilities, one for the full damage and the other one for the halved damage)
