There are several roles in a team, ranks and roles have nothing in common. So it isn't pure logic the carry of your team is always the team leader. So what are roles in a team? Roles represent the thing for what a hero has been made to do or for which it is used. Having only heroes of the same role most likely won't work in a competitive game. Though for every rule there are exceptions.
HardCarry:
This role is most likely preferred by most newbs since in the end they get lots of kills. The main role for a carry is to farm (earning money) all game long until he or she has gotten such powerful items that he or she can carry the team to victory. Carry tend to farm all game long. Though a great carry makes sure he or she does tend to be at every team battle since they have insane damage output in a short time, though if they end up dead they miss out on a lot of farm. Basically a dead carry counts as 2 deaths on your side. Though some heroes are full carries some other heroes are semi-carry since they can also be used for other reasons. Though the difference is a well fed carry out damages a well fed semi-carry. But a carry lacks diversity. Most carry heroes should be getting an Black King Bar against Chain stunning etc. Though not every carry can make good use of it such like image heroes.
There's a big difference in carry's some carry shine throughout early game (Traxex, the Drow Ranger) and other shine throughout late game and are able to solo teams (Terrorblade, the Soul Keeper).
Nuker:
These guys shine mostly throughout early game, they're entire role means throwing spells at the head of your enemies. This give shitloads of damage in a short time thus useful for finish your enemies of or harassing them. Nukers or well most likely AoE nukers are used for pushing creeps and counter pushing them. And to deal shitloads of damage in a short time during team battles. Nukers need to take in to account to be able to do fast calculating and always keep track of the enemies health pool since it sucks to have a runner even though they miss out on experience and gold its most likely better to kill them of Though sometimes making them return to the fountain pays of better.
Ganker:
These guys roam around and kill of the enemy. At least that's their main role most likely they're Runewhoring as well. These guys have either shitloads of nukes disables or can be sneaky bastards like Rikimaru. Dragon knight is originally a gank hero though due to his ultimate he became one of the top tier semi-carries. Gankers are second in line in fights, they deal damage but if it’s a pick between you doing another salvo or the guys who we're farming in that lane if it’s a kill finishing nuke, decide who needs more farm if it’s just an additional nuke save up your mana since you'll need it for the next couple of ganks, and your team's carry has most likely a way more expensive core. Since it's your job to kill enemy that are scattered all over the map you'll walk a lot around the map thus it's time convenient if you place a ward every now and then.
Battle Initiator:
These guys most likely farm up a dagger blink in stun and try to deal as much damage as possible. A good initiator blinks in stun deals damage, a great initiator blinks in out of nowhere and disables the entire enemy team. Though they have a lot in common they do tend to be more team fight focused. These guys are way up front in fights, since they have mostly close combat stuns they will mostly be beefy to so they will go tanking after wards.
Tank:
These guys tend to take up as most them as possible in order to get their allies away from the focus, though most likely tanks can be ignored in team fights. These guys are in front of the fights absorbing damage and towers when you retreat the others should to since they will be taking damage after you've left.
Support:
This is most likely the most underrated and hardest job in the team. Its their job to keep the team alive, also they should make sure your team has map awareness, due to warding etc. sight on the enemy is very useful. You'll be Often called a wardbitch. You can divide these heroes in to two semi groups Disablers and Healers. Support heroes lay back and join in later in fights, after using their skills they lay back again.
How to get a team, you start by either getting your friends and your friends their friends and their friends etc. addicted, or play pubs and keep in touch with the players you've cooperated on either BNET or Garena. Tend to play more games together wait 5 minutes for each other to play the same game together instead of just rushing into a new game. It is true individual skill is also required but you can also work on your individual skill while playing with friends and mates and their friends and mates. Or you can search for external member on the forums and try a few game with them. After you've made contact with a small group of players who play "the same way during the game". Out of several people there experiences it is smarter to first join a few clan and learn a lot about dota and advanced strategies than just being to stubborn and starting a new clan. It's easier to learn the way of the competetive area when some clan can guide you into it, than having to find it all out by yourself the hard way. You want to organize everything in order to increase the teamwork. It's better to gain experience about the competetive scene first before leading a Clan.

Make sure everybody has a working warcraft III TFT, yea I know most people did not start with an legal version, however if you can get an legal version I do recommend it. The next thing to do would be downloading Garena its easier to join the same games with this program since it shows you where your friends and mates are located and playing. After downloading and registering Garena. Try to make some appointments about times and rooms when you guys play a game together, though take into account if you say you'll be starting at 9:00 PM you'll be starting probably at 9:15 PM. Since your entire team needs to join the same room and get everything "fired up".
Ranks inside the team:
Team captain: The guy who makes the fast decisions
3x Lane master: Each lane has 1 most likely the guy with the first stun, this guy decides what to do. Range preferred though 1st stun > Ranged if you have to decide.
1/2x Lane Slave: Each duo lane has 1 these guys are most likely melee, or jungling/ganking. They have follow up stuns like Light Strike Array from Lina or Earthquake from Leshrac. Babysitters are most likely also Lane Slaves
Shown in the Picture below
This is most likely the most important part of good teamwork: communication. Talk with each other, explain your plans before doing them. Tell your team your lane is missing heroes. Make rules on how you communicate, calling misses = MISS SVEN or MIA SVEN BOT HALF HP make clear rules about how you guys say things, some say
MIA(=Missing In Action) others say miss, make sure everybody understands the shorter ways of saying. Be clear fast, but tell as much of the necessary information. How much health does the enemy have remaining, where it most likely is heading, does it have enough mana for a nuke/disable. During laning make sure your lane partner knows if he can count on an secondary stun/nuke. Etc. you can act on your own but first let your team know what you're planning to do.
Become one mind with your teammates. In the beginning you'll might waste to much time getting your teammates attention. After a while an ally will recognize your moves and join in adequately without you having to say it anymore. But ALWAYS CALL MISSES even though you think the already know they might have not known and end up dead because you thought they'd knew already. It's the most common death in DotA.
To have some logic in the team it would be wise to give someone the leader position since democracy is to slow to make fast decisions. Although besides in game I do recommend democracy since everyone's opinion needs to be heard in a team it’s the best to be even outside the games. Though inside a game each lane has a leader and the entire team also has a leader. Whom to give the right to call the shots should be the one who has the most spare time to watch and control the map. Most of the times the solo hero will be the team leader but on these there are several exceptions and can't really be seen as a rule. In lanes the lane master will probably be the one whom has to initiate the kill. When ganking, the one that has joined in on your gank should still be placed under the lane master in order to succeed in the kill most times. So it isn't illogical that the team leader takes order from the lane master.
To accelerate communication it would be wise to add in some Audio programs with Headphones, some prefer push to talk other hate since it takes time to click the talk button. It's something you'll need to experience in order to find out what suits you but it’s the best everybody keeps silent unless they have information about the enemy or telling that they are going to use the chicken, or regenerating at the base. Some of the programs you might want to use so you can talk to each other are
There are several more, though I think these are the most common used programs.

2 Desolators on a single team won't work, which needs which items you should know which items to get on which heroes. Every line up there's a different way of building a hero but as well there is different way of play style. Which skills to learn on which levels etc. Settlement
