This is the only guide you will ever need to play Juggernaut. I also call it the shortest yet most thorough guide ever. If you disagree, well you sir are wrong.
I will teach you how to kill as many people as possible while minimising deaths. Because that is what juggernaut does: kill people. If you are looking for a pusher or support hero then you are in the wrong place.

Bladefury is the most powerful ability jugg has. How well you dominate will depend on how well you use bladefury.
Step 1: Select your target
Bladefury does 300/375/450/525 real damage to your enemy (which is a lot early game). That means that if they have less hp than that, they'll die. If you have omnislash up, then make a quick estimation.
Don't try to kill heroes with blink, windwalk or other escape mechanisms. Don't try to kill heroes who have a lot more movement speed than you (razor?). You'll fail. Don't go after a big fat centaur unless you are very sure you can kill him. Good targets are int/agi heroes with no stuns. But nevertheless, the rule is, as long as you can take him down without dying, go for it.
Step 2: Get near to your target
Getting near to your opponent is not easy if they are not fuckwits.
There are 3 options:
1. Emerge from fog
2. Get ally to help
3. Wait for them to run into you
4. Get a haste/invis rune
I prefer 1, 2 and 4. You?
Lane with a hero that has a stun or a slow, which also doubles as a nuke. Offense, people, offense. Rylai, visage, venomancer, etc.
Also learn how to surprise people from fog. Hide in nooks and crannies, ambush them from the secret shop, keep a salve (and eventually bottle) AT ALL TIMES early-mid game so you can disappear, heal quickly while your enemies overextend themselves, and take them out from the back.
Some opponents have INSTANT stuns that stop you completely. In which case, hit fury as soon as they gain sight of you. You, having grown a brain, will factor in the lost seconds of damage accordingly -- that is to say, you won't deal full damage.
Some opponents have slows that can be dispelled by fury. I won't list them out for you, because it is too tedious. Learn the game.
In which case I like to bait them into using the spell before hitting fury.
Some opponents have projectile stuns. Hint: Ogre's fireball is not a projectile stun. Storm bolt is. You can dodge these spells if you time it right. Hit fury as soon as you see the bolt in the air...you have more allowance the further your target is. If the enemy sven is less than 300 cells away from you, for fucks sake, just hit fury.
Some opponents have no stuns, no slows, and no escape mechs. These are what we call suckers ripe for the picking. Gank them, and gank them hard.

Hit fury. Remember, it dispells runes.
Step 4: Press "M", left click on target
Step 5: Wait 5 seconds; do nothing unless:
1. Enemy's already dead.
2. Counterganks arrive. In which case you're on your own - decide whether to go for the kill, or just run.
3. Enemy moves into fog and you completely lose vision. In which case resume controlling your hero like usual. This -IS- very rare.
FOR FUCKS SAKE DO NOT RIGHT CLICK ON THE GROUND/HERO OR ANYTHING ELSE WHATSOEVER. If you don't believe me then try it yourself.
Step 6: JUSTICE! (+300 gold)
Now let me teach you how to escape ganks with bladefury:
Hit Fury, Teleport back to base.
Only works if enemy has no ultimate stuns (i.e. primal roar, fiend's grip) or bashes (pray hard they don't go off).

Healing Ward is not the most synergistic skill but it's pretty good lategame when you need to push or just to gain the extra edge in battle.
In a 1 on 1 against another melee hero, you can often gain the advantage by going into a nook, leaving a small space behind you, and planting the ward there.
In a 5 on 5, you can place the ward either behind the team and hope it stays alive or you can choose to place it right in the middle of all the confusion where it may stay unnoticed for a few seconds. Either way, you can get maybe 4-5 seconds of heal (plus a few more as the buff disappears) which can make a big difference.
If you just won a large battle then get your motherfucking tank to tank the tower while you place the ward. It is guaranteed the tower will not kill him. And then you can rax.
When you Roshan, you might also want to place to ward as need dictates.

No explanation needed.

Contrary to what people think there's not a lot to explain about omnislash. It's omnislash. It doesn't affect ethereal units, it does physical dmg bypassing avatar, etc.
It does 600/1000/1600 dmg and 1000/1400/2000 with Scepter.
You also attack as per usual while in omnislash, so factor that in. You can also use item spells (dagon, manta, etc.) while slashing.
Lastly, all orbs still apply to the normal attacks.
Use it when there are no creeps around.
I won't waste time telling you about all the stupid "combos" you can do. Wtf, fury, omni? No shit, sherlock. One thing to note however is that you can omnislash first and then hit F right after to kill a target like a pseudo blink strike to close the distance. Just make sure no other targets are available.
How to teamfight:
Going in with bladefury first is dumb. Judgement, people, judgement - if you are likely to take more damage than you dish out in fury, then for fuck's sake, don't. If you right click your targets in bladefury, you will apply the deso debuff - if you have a deso - to your targets, which sets them up nicely for an omnislash.
Juggernaut is a sweeper kind of hero - you know, the kind that goes in at the right time and takes all the kills (i.e. the best kind of hero). He does most of his damage through spells early-midgame, but can be a semi-carry-dps hero lategame if farmed enough. He can't compete with the likes of viper, proudmore or dusa in terms of carry ability, so try to crush them before it gets to this stage. Don't let it drag into late-late game - healing ward really speeds up raxing.
Don't tank torrents and echo slams unnecessarily...try to wait for the big ults to land, or time your fury through them, dispose of creeps and then omnislash. Clean up with normal attacks and allied disables. Also plop down a ward as needed to heal your allies.
The last thing you want to do is die. If you're feeding as jugg then you don't know how to play jugg. Don't feed.
Just kill people and move with the team. When jugg has his ult up, he should be actively hunting heroes. If omnislash is not ready, take time to farm, preferably in the jungle.

Treads OR Phase: Treads give more hp, which is the main draw, but you can phase after you hit bladefury which allows you to keep up with fast heroes like nessaj. It is IMPOSSIBLE to say which one is flat out better, so adjust accordingly. More nukes = Treads.
2x Hammers -> Deso OR Pers -> Bfury OR 2x Blades -> Diffusal
Deso is the ideal option because it makes your omnislash that much stronger. Armour reduction amplifies your ult's damage by about 30%. Good?
But in longer games where farming the hammers can be a little hard, go for a Bfury and neutral. Splashing crits are painful. You can build deso later, anyway. Plus you can cleave in omnislash - normal attacks only so don't jizz yourself or something.
In games where farm is VERY tight, go for a diffusal. It helps out in ganks and you can often snare a target so you can kill them. This is my least favourite option.
HP items...Heart, BKB or Aghs
All items are good. In an environment with many disables and where you need to rely on your gradually more powerful physical attacks, get BKB.
Heart is really more of a luxury item if you find yourself being focused often, which may happen.
Aghs is also a good item and improves your ult. In an environment where you find yourself dealing most of your damage through your ult, then buy this.
Extra items: Travels, Butterfly, Dominator, Radiance, Hyperstones, even Lothars
Now this is when it gets a little overboard. Naturally, there is no best item, but take your pick depending on what you need. Balance damage and EHP. These are luxury picks and will usually not appear in your inventory. If you don't have travels lategame then you're a retard.
That's all. Now if you put my ideas into practice and still fail to murder people, then it could be a sign that you might be better off selling corn dogs at the mall.
